
The failure of Novo Nordisk’s ziltivekimab in a pivotal clinical study has caused significant concern within the biotechnology sector, casting doubt on emerging strategies for treating cardiovascular conditions by targeting inflammation. The drug was designed to block interleukin-6 with the aim of reducing high-sensitivity C-reactive protein levels and thereby protecting heart health. However, recent results from the Zeus trial indicated that while the medication successfully lowered these inflammatory markers as intended, it failed to deliver any measurable benefit for patients suffering from chronic kidney disease or atherosclerotic coronary artery disease.
The study enrolled over 6,000 participants who had previously experienced heart attacks or suffered major blockages in their blood vessels. These individuals were randomised into two equal groups receiving either the investigational drug or a placebo and followed for up to four years. Although adverse event rates remained similar between both cohorts, including serious infections known risks associated with IL-6 inhibitors, there was no difference observed regarding mortality among those treated with ziltivekimab.
Martin Holst Lange, Novo’s science chief, had previously described the project as a high-risk venture despite its potential across three indications. He stated that the findings would inform ongoing research and development efforts for patients facing substantial unmet needs in cardiovascular care. The implications of these results extended beyond Novo Nordisk itself, causing share prices to tumble for other companies such as BioAge Labs, Neurocrine Biosciences and Neumora Therapeutics which are pursuing similar mechanisms to lower inflammatory markers.
Analysts have noted that the lack of clinical signal alongside reports of serious infections challenges the established role of high-sensitivity C-reactive protein in predicting heart protection. This development suggests that targeting inflammation alone may not be sufficient to improve outcomes for patients with existing cardiovascular disease, potentially altering investment strategies and research directions across multiple biotech firms working on novel therapies.
